Children’s Information and Permission Forms

There are three important permission forms the conference planning team must receive with each registration:

  1. The Media Release Form is for any adult and every child. This includes professionals, parents, guardians, grandparents, other adults, and children.

  1. The Child Background Information Form must be completed for each child you are registering to attend the 2009 Families Connecting with Families International Conference, including the child with a visual impairment. We need this information to plan childcare and our educational programs.

  1. Finally, if you plan to take advantage of the onsite childcare, family outing and Paralympics on Saturday, you must complete an Activity Permission Form for each child who will be registered.
